Adding your providers to your “My Health Team” app.

There are two ways to enter your providers into is your app:
  • Automated adding.
Your clinic may have an ipad or tablet with a “library” of all providers and their pictures and contact information. If so, they just “airdrop” your providers into your phone once you’ve put your phone on WIFI and Bluetooth and the empty app is on your phone. This is the easiest way to do it so before adding manually, ask if your health center has this technology.
  • Manually adding.
    1. Take a picture of your provider or get one from the web. The medical center will often have them on their website or through a search engines’ “images” feature. If you are getting it from the web, just right click on the picture and save to your ipad, tablet, iPone, or other smart phone camera.
    2. Open the app and click on the plus sign on right upper corner. THIS IS HOW YOU ADD PROVIDERS.
    3. You should see a grey depiction of a person’s head as well as blanks for adding the provider’s name, area of expertise, phone numbers, and email.
    4. Click on the grey depiction of the person’s head and you will be asked if you want to take a picture of your provider or add one from your phone. It’s up to you. If you downloaded one from the web, click on add from my phone. Choose the photograph from among the others on your phone and then resize if needed with your finger. Then press “done”
  • The screen you come back to should have your healthcare provider’s face.
  • Now type in the following:
  1. The providers name
  2. The type of care they provide (e.g., primary care, cancer chemotherapy, heart care, pulmonologist, nutrition support, surgeon, nurse practitioner, chemotherapy nurse, radiation treatments)
  3. Their scheduling phone number
  4. Their emergency phone number, and
  5. Their email if they have one.
Often this information on is their business card if they have one or their secretary or nurse may know. 
  • Then be sure to click “SAVE”, which is low on the screen. 
  • You can add more providers. If you need to edit content press edit. If you want to delete a provider press edit. If you want to change the order of the list of providers press edit.
  • If you are have trouble, please email and someone will get back to you within 24 hours to help you to get your health team into your phone. If you are every in an ER or hospital that is unfamiliar with you, please show them this app so that they may get in contact with your providers to be sure that you get the best and most informed healthcare possible.
